Boone Park Elementary Flu Clinic:

Our school wide flu clinic for BPE students and staff, through the Arkansas Department of Health, will be offered on Tuesday, October 21st. Your student will be bringing home a flu packet today. If you would like for your child to receive a flu shot at school, please complete this packet in its entirety. There will be a form from the Arkansas Department of Health that will need to be filled out ON THE FRONT AND BACK and signed by the guardian. There will also be a half sheet form from NLRSD that will need to be filled out. BOTH OF THESE FORMS MUST BE COMPLETED AND RETURNED TO THE NURSE BY MONDAY, OCTOBER 20TH AT 8:00 A.M. in order for your child to receive a flu shot on Tuesday, October 21st. Forms cannot be accepted on the day of the clinic.

If you do not want your child to receive a flu shot, then no action is required on your part. Students will only receive a flu shot if both forms are completed and returned. Please contact Nurse Gray if you have any questions. Thank you!

All medication that is not picked up by Friday will be destroyed or discarded, per district guidelines.
